eGift Card Terms & Conditions

1. Issuer

eGift cards are issued by PV Medispa & Weight Loss, LLC (“PV Medispa,” “we,” “our”), located at 8008 East Yavapai Road, Suite C, Prescott Valley, AZ 86314. All cards are tied to and redeemable only at PV Medispa, Prescott Valley.

2. Purchase and Delivery

eGift cards are sent by email to the recipient address provided at purchase. Delivery occurs within minutes of payment but may take up to 24 hours during high-volume periods. The purchaser is responsible for providing the correct recipient email; we cannot recover or resend cards delivered to a mistyped address. If the recipient does not receive the card after 24 hours, contact us at 928-910-8818.

3. Promotional vs. Purchased Gift Cards

Purchased gift cards are cards bought for cash, credit, or other payment. These follow federal CARD Act rules and Arizona law (A.R.S. § 44-7402): valid for at least five (5) years from the date of issue, with no dormancy or service fees during the first twelve (12) months.

Promotional gift cards (for example, a free service-credit card given as part of a promotion or campaign bonus) are gifted at no cost and may carry shorter expiration windows and additional restrictions, which will be clearly stated on the card itself.

4. Expiration

Purchased eGift cards do not expire for five (5) years from the date of issue. Promotional cards expire on the date printed on the card. The Mother’s Day 2026 promotional bonus (free take-home lip hydrator with a Berries & Bubbly facial) must be claimed at the time of the booked appointment and is not redeemable for cash, credit, or substitution.

5. Redemption

eGift cards can be redeemed in person at PV Medispa, by phone at 928-910-8818, or through our online booking system at booking.pvmedispa.com when the option is available.

eGift cards may be applied to any PV Medispa service or retail product except: third-party prescription medications, medication shipping fees, lab fees billed to outside laboratories, gratuities (which may be paid separately), and items where applicable manufacturer rebate or financing terms exclude gift-card payment.

Cards may be used in full or in part. If a partial balance remains, it stays on the card and can be used toward future services until exhausted or expired.

6. Combining With Other Offers

eGift cards may be combined with most standard promotions and packages. They may not be combined with the Mother’s Day 2026 single-visit promotional pricing on the Berries & Bubbly facial ($120 with bonus lip hydrator), other introductory or first-visit discounts, or any offer that explicitly excludes gift-card stacking. We reserve the right to determine eligibility for stacking on a case-by-case basis.

7. Refunds, Exchanges, and Returns

eGift cards are non-refundable, except where required by Arizona law. Per A.R.S. § 44-7407, cards with a remaining cash balance of less than five dollars ($5.00) may be redeemed for cash upon request at the time of partial redemption. Cards purchased in error within twenty-four (24) hours and not yet redeemed may be canceled and refunded by contacting 928-910-8818.

8. Lost, Stolen, or Unauthorized Use

eGift cards are treated like cash. If a card is lost, stolen, copied, forwarded, or otherwise used without authorization, PV Medispa is not responsible for replacing the value. If you suspect unauthorized use, contact us immediately so we can flag the card code; we can replace remaining balance only if the card has not yet been redeemed.

9. Transfer

eGift cards are transferable. The recipient on the original delivery email may forward or assign the card to another person at their discretion. Cards cannot be resold for commercial purposes.

10. No Cash Redemption

Except where required by Section 7 above, eGift cards may not be redeemed for cash, used to purchase other gift cards, or returned for store credit greater than the unused balance.

11. Sales Tax

Sales tax is not charged at the time of gift card purchase. Tax is applied at redemption based on the items or services purchased and the tax rate in effect on the date of redemption.

12. Privacy

Email addresses, names, and payment information collected during gift card purchase are subject to the PV Medispa Privacy Policy at pvmedispa.com/privacy-policy. We do not sell or rent purchaser or recipient information.

13. Changes to This Policy

PV Medispa may update this policy at any time. Changes apply to cards purchased after the policy update date. Cards purchased before an update remain governed by the policy in effect at the time of purchase.
